Community Outreach

Community Outreach is a service which aims to support people whilst they are living in the community and is not part of an accommodation based service. We support people to maintain and building skills, networks, interests, employment and community links. This service does not include personal care. 

If you have lost independent living skills or want to gain skills we will help you achieve this. 
We can support you with:
  • Maintaining your tenancy
  • Supported Employment & Job Coaching
  • Education and training 
  • Healthy Eating 
  • Physical and mental health and Wellbeing 
  • Meal Prep and cooking
  • Debt management 
  • Budgeting
  • Using public transport
  • Building Community Links
  • Hobbies and interests 
  • Maintaining and gaining relationships with friends and family
  • Managing symptoms and behaviours 
  • Medication management support (but not administration)
